TIPS IN FILLING THE FORM
  • Name, it's better if you use your online name rather than keep changing your name each time you requested from us.
  • Email given must be your PRIMARY email. It means that the email that you opened most than others.
  • Website or (proof of identity online e.g.: Facebook, Twitter, Tumblr) URL is a must.
  • Celebrities are who / what do you want to feature. It's better if you provide us the pictures you wanted us to use.
  • Please choose your [Type of Request] correctly!
  • It might help me a lil bit if you give the color code if you have specific color that you like. e.g: #238JFQ
  • Wordings is any (non-taboo or related) words you want to be put on the graphic.
  • Any additional info or anything related to your request, please put it there and we appreciate it if you do not confuse our fellow designers. What's three rows one coloum? Check your spelling, do your homework. (oh, and we call it "sidebar" not rows) ^_^

Have a nice day (^o^)/

P/S: If you want your site layout to have two or three columns, sidebar on the left side or right, please mentioned it in the form okay? I do not do navigational layouts. If you want me to design the layout, I can do it for you but for layout coding (for navigational layouts), please request it from Chazzie. And oh, PLEASE, PLEASE, PLEASE follow the rules (^_^)v
